    This was my fist year of Specialist Registrar training on the South Coast Training Programme in Oral and Maxillofacial Surgery. At Chichester I consolidated experience in dentoalveolar surgery, facial pain and oral medicine. As part of the trauma team we managed major traumas according to the ATLS principles. There was comprehensive experience in the management of facial fractures, tracheostomies, soft tissue injuries, orthognathic planning and surgery. I had exposure to surgical dermatology, salivary gland surgery, major head and neck surgery and reconstruction.

    The second year was in Portsmouth and consolidated the training received in Chichester. I completed a micro vascular course and was then able to perform anastomoses for free flap reconstructions. I raised several radial forearm free flaps and carried out neck dissections under consultant supervision.

    At Poole Hospital I gained experience in cleft lip and palate surgery, orbital trauma and temporomandibular joint surgery. I had further exposure to the management of head and neck oncology including cutaneous malignancy. I raised several free flaps that included fibular osteocutanous flaps, and assisted with several jejunal and gastro-omental flaps.

    The fourth year of the rotation was at Southampton University Hospital. This has the regional craniofacial unit and experience was gained in trauma and neurosurgical access. Part of the year was spent at Salisbury General Hospital where secondary cleft surgery is undertaken. There was also experience in surgical dermatology, salivary gland surgery, head and neck oncology and reconstruction. In September 2002, I passed the Intercollegiate FRCS (OMFS) examination.

    I then spent a year out of programme as a Head and Neck Fellow to Professor CJ O’Brien, Dr K Shannon and Dr A Clifford at the Sydney Head and Neck Cancer Institute. This provided excellent experience in the multidisciplinary management of diseases of the head and neck. I received invaluable tuition in micro vascular techniques, parotid, thyroid and parathyroid gland surgery. In the fellowship year, I raised 37 free flaps, which included radial forearm, fibular, rectus abdominus and latissimus dorsi. I was involved with several research projects, teaching junior doctors, medical students and nurses, and I organised the monthly journal club.

    Locum Consultant in Oral and Maxillofacial Surgery

    I completed 7 months as a locum consultant in OMFS with subspecialty interest in head and neck oncology. I had a weekly all day operating list and in most weeks had a patient who required major resection and microvascular reconstructive surgery. I also presented patients at the weekly head and neck multidisciplinary meeting and clinic. There was a regular on call commitment for patients with maxillofacial trauma.

    I completed 3 months as a locum consultant in OMFS with subspecialty interest in head and neck oncology. As with the John Radcliffe Hospital I had a weekly all day operating list but this was shared to provide two team operating for patients who required major resection and microvascular reconstructive surgery. I also presented patients at the weekly head and neck multidisciplinary meeting and clinic. There was a regular on call commitment for patients with maxillofacial trauma and a comprehensive teaching programme for junior staff.

    Consultant in Oral and Maxillofacial Surgery with specialist interest in head and neck oncology

    St George’s Hospital provides the regional Maxillofacial services for South West London and a population in excess of 2 million people. The Head and Neck oncology service is multidisciplinary and meets weekly to discuss the management of patients. I am one of four maxillofacial oncology consultants who are part of a team providing ablative and reconstructive surgery. The case mix on the operating lists includes at least one major ablative and microvascular reconstructive procedure each week, salivary gland surgery, oral and maxillofacial trauma implantology, bone grafting, surgery, complex dento-alveolar and surgical dermatology. There are also four more Consultant Maxillofacial Surgeons who have subspecialty interests in facial deformity surgery.
